If you require a repeat prescription, a social welfare cert or want to enquire about the results of tests you recently completed at the clinic please see the "Online Forms" page in the menu above and submit your request.  We require 48 hours notice for these requests.

If you require an appointment, we ask that you phone the clinic on the number below in order for us to triage your call and screen for symptoms of COVID.  The clinic is extremely busy at the moment and we thank you for your patience when you call us.  If you are calling in relation to an emergency, please dial 112.
